Simple Will for Maine

This is a list of the information that will be entered in the form, which you can gather before you begin.

1. Your Personal Information
• Your Name
• Your Father's and Mother's full names
• Your date and place of birth (including county)
• Your current residence address
2. If married, Your Current Marriage and Spouse Information
• Date and place of your marriage (including county)
• Your Spouse's Name
• Your Spouse's Father's and Mother's full names
• Your Spouse's date and place of birth (including county)
3. Your Children — if any
• Name of each child
• Date of Birth of Each Child
• Relationship to you
• Address of each Child — if not living with you
4. Guardians
• Guardians for any minor or incapacitated children, an incapacitated spouse, or others.
• Only the same primary and alternate guardians are allowed in this form for all persons
• Name and residence city of the primary and alternate guardians
5. Age for Children to Inherit
• Select age at which children are to inherit — normally 18
6. How you want to give your money & property after your death — Gifts to heirs
• You may give specific property, money or other items to anyone you may desire (as allowed by law)
• You then must decide who is to receive the residue (what's left) of your money & property, i.e. family members, a church or other charity, or any other lawful person or entity
7. Pets
• You are allowed to select whom you want to give your family pets to along with a specified amount of money & property for their care.
8. Personal Representative
• You must select a personal representative and should select an alternate personal representative to carry out your wishes under your will
