- Guardians for any minor or incapacitated children, an incapacitated spouse, or others.
- Only the same primary and alternate guardians are allowed in this form for all persons
- Name and residence city of the primary and alternate guardians
- Age for Children to Inherit
- Select age at which children are to inherit — normally 18
- How you want to give your money & property after your death — Gifts to heirs
- You may give specific property, money or other items to anyone you may desire (as allowed by law)
- You then must decide who is to receive the residue (what's left) of your money & property, i.e. family members, a church or other charity, or any other lawful person or entity
- Pets
- You are allowed to select whom you want to give your family pets to along with a specified amount of money & property for their care.
- Personal Representative
- You must select a personal representative and should select an alternate personal representative to carry out your wishes under your will
- Name and residence address of the primary and alternate personal representatives
